Types Of Breast Implants Used in Breast Augmentation

In the previous article on breast enhancement, we discussed what augmentation mammoplasty was, what the basic procedure was, and who usually opted for this procedure.

In this article, we will discuss the types of implants used in augmentation mammoplasty.

Augmentation Mammoplasty – The Types of Implants

There are three types of breast implants – silicone, saline solution, and cohesive silicone gel. Basically, augmentation mammoplasty involves the placement of a hollow shell (called the lumen) made of silicone and filled with an appropriate substance, usually a solution or a gel. A breast implant primarily consists of three parts – the lumen, the filler, and a patch that covers the aperture through which the solution is filled into the lumen.

Nowadays, all breast implants have to be FDA-approved. FDA-approved silicone implants are made of polymer shells and contain a saline solution inside.

Augmentation Mammoplasty – Silicone Implants

Silicone implants involve the usage of silicone gel as the filler inside the lumen. In its early days, augmentation mammoplasty saw the extensive usage of silicone gel as a filling inside the lumen. This was prevalent practice till 1992, when the FDA banned the usage of silicone as it was harmful to the body. Patients who had these implants were found to have problems related to implant leakage and rupture.

Patients filed suit, and while some of these were not upheld, there were others that were. As a result, silicone gel implants were yanked off the marked in 1992. Nowadays, they are not used too often. Usually, nowadays only patients who are part of medical studies and research are allowed the usage of silicone gel implants.

There are other instances when a woman is allowed usage of silicone gel implants. This is usually the case if she has thin breast tissue. Another circumstance when a woman can have silicone gel implants is after breast reconstruction surgery. A woman can also have silicone gel implants if she has had a saline implant that has not gone too well. In such instances, the implants may ripple. (Rippling is a condition involving wrinkling of the implant. It occurs due to the filler inside the implant moving around.)

Augmentation Mammoplasty – Why Silicone Implants were Banned

Augmentation mammoplasty is an implant procedure that involves placing an element inside the body. The body is very sensitive and any foreign object inside it, if not properly inserted, can cause discomfort at the least, and harm at the worst.

The problem with implants is that there is a chance that the solution inside the lumen may leak. If that happens, it is critical to ensure that the substance that is leaking out is something that the body is used to. In the case of silicone, this became an issue. Silicone is not something that the human body is used to, and the leakages were found to cause serious problems.

With saline solution as the filler, the problem was solved. The body is used to salt water, which is what a saline solution actually is, and could therefore easily absorb it.

Augmentation Mammoplasty and Saline Implants

Saline gel implants basically contain a saline solution as the filler inside the lumen. Most saline solution implants require the filler to be filled during the surgery. However, pre-filled saline implants are available nowadays, but are under review by the FDA.

They became very widely used after the silicone gel implants were banned. Saline implants initially faced one criticism – they were apparently not as soft as the silicone gel implants. This has been addressed in the latest versions in the market.

A saline implant can be between 120cc to 850cc.

Augmentation Mammoplasty and Cohesive Silicone-Gel Implants

Cohesive silicone-gel implants have been widely used in Europe since 1995, and have been approved for use in Canada as well as Australia. However, in the US, they are still in the clinical trials state. Cohesive silicone-gel implants are different from the silicone implants, and these differences are what will most likely make it the most popular implant form.

The first difference is in the shell. While it is still made of silicone, it is multi-layered. This has been designed to prevent the silicone filler from spilling out. Besides, the gel inside is also thicker and more cohesive. This is to ensure that even if the implant has a tear, the gel remains inside in its original shape and does not spill out. These implants are also designed to prevent rippling from happening.

The disadvantage is that this form of implant requires a larger incision, made usually under the breast or around the areola.
